Support at Home Program (SaHP)
The Support at Home Program is a new national aged care system, designed to make it easier for older Australians to stay safe, comfortable, and independent at home.
It combines a wide range of services, equipment, and home modifications into one simple system that is tailored to your unique needs.

Pricing caps (starting July 2026) to ensure fair and transparent pricing across all providers.
10% Care Management fee, which supports ongoing care planning, check-ins, and guidance.

Support at Home includes a dedicated pool of funding for essential equipment and modifications to keep you safe and independent.

Eligibility

Available to all participants based on assessed needs.
How It Works

You must use any unspent Support at Home funds first.

Funding amounts:

Assistive Technology: Up to $15,000, with flexibility to go higher if needed.

Home Modifications: Up to $15,000, no flexible limit.

Includes OT assessments and wrap around support.
This helps you access important safety equipment without needing to save up over time.
SHORT-TERM CARE PATHWAYS
These pathways provide extra support during times of greater need.
Restorative Care

Helps you regain strength and independence after illness, injury, or hospital stays.

Provides targeted allied health support.

Available for up to 12 weeks, with a 4-week extension if required.
End-of-Life Care
​

Supports people with a life expectancy of around three months or less.

Helps you remain at home for as long as possible.
-
Offers up to $25,000 in additional funding and services for up to 16 weeks.

Your level is determined by an assessment from the Aged Care Assessment Team (ACAT) using the new Integrated Assessment Tool.
PARTICIPANT CONTRIBUTIONS
Depending on your income and the type of service, you may be asked to contribute a portion of the cost.
All contributions count toward a $130,000 lifetime cap, which also applies if you move into residential care.

CAPPED CARE MANAGEMENT
Care Management is an important part of your package and ensures you receive the right support at the right time. Your 10% Care Management fee includes:

Developing and reviewing your personalised care plan

Regular check-ins to monitor your wellbeing

Advice, education, and support for you and your family

Clinical oversight and help adjusting your services as your needs change
This ensures you always have expert guidance throughout your care journey.
UNSPENT FUNDS
If You Already Have a Home Care Package:

All your unspent funds will transfer across to the new program.

You can still use these funds for extra services, equipment, or home modifications.
Under Support at Home:

Each quarter you can carry over up to $1,000 or 10% of your quarterly budget, whichever is higher.

This cap applies only to funds accumulated after joining Support at Home.
-
Existing Home Care Package unspent funds are not affected.
